IBID Follow Up Fields

Follow Up (first seen by any MDT member after discharge
Follow Up Location - Enter in to the free text field the primary site for initiation of post discharge follow up.
Follow Up Date - Enter from the drop down calendar the date of the first follow up appointment, to occur following discharge as an in patient.
Follow up time - Enter the time of the day of first follow up appointment was to occur following discharge as an in patient.
Post Discharge
Return to Work/Education Date - The date on which the patient returned to work or full-time education.
Discharged from Service - Tick box. Ticking this bow will generate further fields indicating date discharged.
